Registration Form  

Registration to the Conference and Tutorials must be made by sending the Conference Registration Form (including extra page charges) and remittance of payment (by mail or fax) directly to:

Conference registration fee include:
* One copy of the conference proceedings
* Two Conference Lunches
* Welcome Reception on October 25
* Invitation to the Conference Banquet on October 26

Tutorial registration fee includes registered tutorial notes. More than one tutorial can be registered by paying separate registration fee.

Note: All authors need to send copyright form to the Registration Chair Prof. Hongyi Wu along with the registration form.

Written requests for refunds must be received by the MASS-2004 Conference Registration Chair (see address above) no later than October 1, 2004. Refunds are subject to a $75 service charge. Registered participants who fail to attend or notify the MASS Registration Chair of cancellation by the above date are subject to the full fee.
